Remote Registered Nurse Care Manager – Primary Care (URGENTLY HIRING)
Position Overview
Join a dedicated healthcare team in a role that focuses on providing comprehensive and coordinated nursing care to an assigned panel of patients. You will collaborate with Veterans Affairs and community services to enhance health promotion, disease prevention, and address the acute, chronic, and long-term health needs of Veterans, specifically in the Oncology and Hematology departments.
Contract Information
- This is a telehealth role supporting patients from the Durham, NC Veteran Affairs Medical Center.
- The initial contract duration is one year, with a potential extension up to a total of two years.

- This position is eligible for remote work, provided you live in one of the 50 U.S. states or the District of Columbia.
Key Responsibilities
- Assess patient healthcare needs and develop or maintain individualized care plans.
- Respond to significant changes in patients’ health conditions with timely interventions.
- Coordinate care and provide direct patient care while educating patients and their families on health management.
- Facilitate the admission, transfer, and discharge processes for patients.
- Serve as a Patient Aligned Care Team (PACT) leader and mentor colleagues and students to foster their professional growth.
- Educate patients and families about health conditions, treatment plans, and medication management.
- Provide comprehensive care coordination and case management, with a focus on Women Veterans, and support vaccination initiatives and clinical services.
- Perform accurate physical assessments and triage patient needs, making informed, independent clinical judgments.
- Organize and prioritize workload while collaborating with community resources and other healthcare providers to ensure coordinated care across all settings.
Qualifications
- Bachelor of Science in Nursing.
- Graduation from a college or university accredited by the National League for Nursing Accrediting Commission (NLNAC) or The Commission on Collegiate Nursing Education (CCNE).
- Basic Life Support (BLS) and Advanced Cardiac Life Support (ACLS) certification from the American Red Cross or American Heart Association.
- Minimum of 3 years of experience in case management.
- Minimum of 3 years of experience in hematology/oncology.
- Active, unrestricted RN license in any state or commonwealth, with applicable licensure for your practice location.
- U.S. Citizenship is required.
- Proficiency in EMR/EHR systems and technology.
- Successful completion of any necessary federal background check and compliance with VA credentialing policies.
Additional Notes
- This position is contingent upon contract award, client approval, successful background investigation, and the ability to obtain and maintain a Public Trust/High Risk clearance.
